About Your Appointment

In preparation for appointments, Wound Specialist Services requires the following documents to be uploaded through our secure client portal:

Patient Intake Form

– Photographs of the wound(s)

– Brief description of the problem and current treatment

– Health summary

– Medication summary

– Results of any recent investigations (where relevant)

During the appointment, the Wound Specialist Services practitioner will obtain a detailed history to help determine the wound aetiology based on history, clinical signs and symptoms and results of any investigations, provides education about the problem and recommendations for treatment.

If required, investigations or referrals may be initiated. If there is a need for a face-to-face consultation, this will be discussed at the time of the appointment.

There is always plenty of time for question and discussion or to demonstrate techniques.

Following the consultation, Wound Specialist Services provides a full report within 24-hours to the referrer and other relevant care providers including the GP to strengthen communication and collaboration.

A follow-up appointment is usually recommended about two weeks after the first consultation. The purpose of the appointment is to monitor progress, to evaluate outcomes and review the appropriateness of any recommendations that may have been implemented.
